چکیده

Abstract BACKGROUND Integrative brain tumour diagnostics indisputably requires comprehensive reporting of molecular markers. The 2021 WHO classification central nervous system (CNS) tumours substantially increased the set markers for routine evaluation, with greater significance to DNA methylation analysis in diagnostics. Limited by investment and batching, smaller labs clinics might suffer major delays delivering clinical decisions. To make precision accessible, we introduce an integrated computational histopathology adaptive nanopore sequencing workflow next day CNS METHODS We used CNS-CHiP- a multitask deep transfer learning model predict key alterations from H&E stained slides. For further characterisation subtyping, predictions formulate custom panel each patient. Targeted analyses were performed using Rapid-CNS2- neurooncology pipeline parallel copy-number, mutational that is flexible target selection no additional library preparation can be initiated upon receipt frozen sections. Sequencing was on portable MinION or GridION. RESULTS demonstrate our diagnostic samples received Department Neuropathology, University Hospital Heidelberg. CNS-CHiP predicted multiple pathognomonic (eg. IDH mutation, 7 gain/10 loss) reasonable accuracy. This provided basic information regarding tumor type instantly. Personalised panels enabled small sizes, resulting low time (up 24h) competitive costs. GPU-accelerated bioinformatics reduced > 24h < 3h. CONCLUSIONS Our harnessing histology-based instruct targeted up initial has potential facilitate results sample collection. combined Rapid-CNS2 thus aims affordable accessible hospitals especially low- middle-income countries.

Targeted next-generation sequencing for the molecular genetic diagnostics of cardiomyopathies.

BACKGROUND Today, mutations in more than 30 different genes have been found to cause inherited cardiomyopathies, some associated with very poor prognosis. However, because of the genetic heterogeneity and limitations in throughput and scalability of current diagnostic tools up until now, it is hardly possible to genetically characterize patients with cardiomyopathy in a fast, comprehensive, and...

Targeted next-generation sequencing for the detection of ciprofloxacin resistance markers using molecular inversion probes

Antibiotic resistance (AR) is an epidemic of increasing magnitude requiring rapid identification and profiling for appropriate and timely therapeutic measures and containment strategies. In this context, ciprofloxacin is part of the first-line of countermeasures against numerous high consequence bacteria. Significant resistance can occur via single nucleotide polymorphisms (SNP) and deletions w...

Molecular diagnostics of a single drug-resistant multiple myeloma case using targeted next-generation sequencing

A 69-year-old man was diagnosed with IgG λ-type multiple myeloma (MM), Stage II in October 2010. He was treated with one cycle of high-dose dexamethasone. After three cycles of bortezomib, the patient exhibited slow elevations in the free light-chain levels and developed a significant new increase of serum M protein. Bone marrow cytogenetic analysis revealed a complex karyotype characteristic o...

Targeted next generation sequencing for molecular diagnosis of Usher syndrome

BACKGROUND Usher syndrome is an autosomal recessive disease that associates sensorineural hearing loss, retinitis pigmentosa and, in some cases, vestibular dysfunction. It is clinically and genetically heterogeneous. To date, 10 genes have been associated with the disease, making its molecular diagnosis based on Sanger sequencing, expensive and time-consuming. Consequently, the aim of the prese...

Rapid Targeted Next-Generation Sequencing Platform for Molecular Screening and Clinical Genotyping in Subjects with Hemoglobinopathies

Hemoglobinopathies are among the most common autosomal-recessive disorders worldwide. A comprehensive next-generation sequencing (NGS) test would greatly facilitate screening and diagnosis of these disorders. An NGS panel targeting the coding regions of hemoglobin genes and four modifier genes was designed. We validated the assay by using 2522 subjects affected with hemoglobinopathies and appli...
